What Continuous Survey Readiness Actually Requires: A Data Infrastructure Checklist
Every health system says it wants continuous survey readiness. Very few have built the infrastructure to actually achieve it. The gap is not ambition or awareness. It is data infrastructure. Continuous readiness is not a mindset shift. It is an engineering problem with specific technical requirements that most organizations have not met.
This article lays out exactly what your health system needs, component by component, to move from cyclical survey prep to always-on accreditation readiness. Why "We Are Always Ready" Is Almost Always False

This is the foundation. Without it, nothing else works.
For health systems on Epic, the clinical evidence for most compliance metrics already exists in Clarity and Caboodle. Timestamps for clinical interventions. Documentation completion rates. Quality measure data points. Patient safety indicators. Medication administration records. All of it is being captured as part of normal clinical workflows.
The problem is extraction. Most compliance teams do not have direct access to this data. They rely on IT or analytics teams to pull it, often weeks after the data was generated. By the time the compliance team sees it, the window to intervene has closed.

Component 2: A Regulatory Metric Library
Having access to the data is not enough. You need to know what to measure and how to measure it.
Each regulatory program defines its metrics differently. CMS Conditions of Participation measure specific care delivery requirements. TJC standards under Accreditation 360 define performance expectations across multiple domains. Leapfrog survey measures track specific safety and quality indicators. IQR and eCQM programs have precise technical specifications for numerators, denominators, and exclusions. MIPS, VBP, HCAHPS, HACRP, and ORYX each have their own measurement methodologies.
A continuous readiness system needs a metric library that maps each regulatory requirement to the specific data points in your EHR that satisfy it. This is where most generic BI dashboards fail. A Power BI developer who does not understand CMS Conditions of Participation will build a dashboard that looks professional but does not answer the questions a surveyor will ask.

Component 3: Automated Compliance Dashboards
This is the visibility layer. It is what your compliance team actually looks at every day.
The dashboards need to show current performance against every regulatory requirement the organization is accountable for. Not a summary. Not an aggregate score. The actual metrics, at the level of detail a surveyor would examine.
For a TJC-accredited hospital, this means dashboard coverage of patient safety standards, infection prevention metrics, medication management indicators, clinical documentation compliance, and every domain addressed in the Accreditation 360 framework. For CMS, it means Conditions of Participation metrics. For Leapfrog, it means each survey section with current performance relative to scoring thresholds.
The dashboards should be built for compliance and quality team members, not for analysts. That means clear status indicators (meeting standard, at risk, below standard), drill-down capability to the department or unit level, and the ability to see trends over time rather than just a current snapshot.

Component 4: Threshold-Based Alerting
Dashboards show you where you stand. Alerts tell you when something changed.
The value of continuous monitoring is not the monitoring itself. It is the early warning system. When a metric starts trending toward non-compliance, the compliance team needs to know immediately, not at the next monthly review meeting.
Effective alerting requires defined thresholds for every metric. Some are binary (the organization either meets the requirement or it does not). Others are trend-based (a metric that has been declining for three consecutive weeks is an alert even if it has not yet crossed the compliance threshold).
The alerts need to reach the right people. A patient safety metric trending downward should alert the VP of Quality and the relevant department director. A documentation compliance gap should alert the HIM team. A Leapfrog measure that dropped below the scoring threshold should alert whoever owns the survey submission.

Component 5: Program-Specific Reporting Automation
Continuous readiness is not just about internal monitoring. It is also about reducing the labor required for external reporting.
Your quality team is measured on IQR, eCQMs, MIPS, VBP, HCAHPS, HACRP, and ORYX. Each program has specific submission requirements, timelines, and data formats. Most organizations build these reports through manual abstraction, a process that consumes significant analyst hours and introduces errors at every step.
When the analytics layer is built correctly on the EHR data, the same infrastructure that feeds your compliance dashboards can also feed your program reporting. The data is extracted once, measured against program specifications, and delivered both to internal dashboards and to external reporting pipelines.

The Implementation Sequence That Works
Organizations that try to build all five components at once typically stall. The sequence that produces results fastest, based on what we have seen in practice:
Phase 1 (Days 1-30): Data source connection and highest-risk metrics. Connect to Clarity and/or Caboodle. Build dashboard coverage for CMS Conditions of Participation patient safety metrics and TJC critical standards. These are the findings that create immediate jeopardy risk.
Phase 2 (Days 30-60): Expand coverage and add alerting. Add Leapfrog survey measures, infection prevention metrics, and documentation compliance. Implement threshold-based alerts for all Phase 1 and Phase 2 metrics.
Phase 3 (Days 60-90): Quality reporting integration. Connect IQR, eCQM, and other program reporting to the same analytics layer. Automate what was previously manual abstraction. This is where the labor savings start compounding.
Ongoing: Maintain, expand, and optimize. Add new programs as requirements change. Update metric definitions annually. Optimize queries for performance. Train compliance team members on self-service dashboard use.
